This website shares general information about the practice and the care it offers. It isn't medical advice, and reading it doesn't create a clinician and patient relationship. That starts when you have a visit.

Emergencies

Don't use this website, the call-back form or text messages for emergencies. Call 911. In a mental health crisis, call or text 988.

Telehealth

• You must be located in Arizona at the time of your visit.
• Some problems need an in-person exam, a lab or imaging. If that's the case, you'll be told and pointed to the right place.
• Visits need a working phone or internet connection. If a connection fails, we'll try to reconnect or reschedule.

Prices and payment

Visits are self-pay. The price of your visit is shown before you book. Prices may change, and the price shown when you book is the one that applies. Medications, labs and imaging are paid separately to the pharmacy, lab or imaging center. As a self-pay patient, you have the right to a Good Faith Estimate.

Prescriptions and treatment

Any treatment, including a prescription, is a clinical decision made during your visit. No specific medication or result is promised.
