Effective October 2026. This policy explains what the Definitive Care and Modern Wellness website collects and how it's used. It covers this website only. What this site collects

If you use the call-back form, you give us your name, phone number, and optionally your email, the best time to reach you, the kind of visit you want, and whether it's OK to text you. If you book online, the scheduling service collects the details you enter there.

Like almost every website, our host keeps basic technical logs, such as your IP address and browser type, for security and to keep the site running.

What this site doesn't do

• No advertising or tracking pixels on our pages.
• No selling or renting your information.
• No health questionnaires. Please don't send medical details through the website.

How your information is used

Only to contact you about scheduling and to answer your questions. If you check the box allowing texts, we may text you about scheduling. Reply STOP to opt out at any time.

Who helps us run the site

• Netlify hosts this website and receives call-back form submissions.
• Tebra (PatientPop) runs the online booking tool on the booking page. It loads from Tebra's servers, may use its own cookies and analytics, and its own privacy policy applies to what you enter there.

We may also share information when the law requires it.

How long we keep it

Call-back requests are kept only as long as needed to schedule your care [Confirm retention period].

This site isn't directed to children. Parents and guardians book visits for kids.
